Project LIVE Case Manager

Work with low-income, vulnerable seniors who need linkage to community resources so that they can maintain an independent living situation. Conduct interviews, refer clients to other community agencies for support services and training; monitor client progress; provide follow-up services and crises intervention if indicated; maintain records; and make reports. Four years of college or a combination of education and work experience in social work, human services, adult education, or related field. Must have a reliable automobile, valid driver’s license and insurance for daily use (mileage reimbursed). Preferred qualifications include two years of related work experience. Salary $32,876+ depending upon qualifications.
